The short answer
The Fragmented unlocks with Whispers in the Walls and lives on Effervo, Deimos. You scan for Murmur Eyes with Atropos Probes and Vitreum scanners to find it, then fight one of three variants and shoot its glowing teal spheres. The harder version, the Fragmented One, needs 60 Eyes on the Steel Path. It drops Melee Arcanes.

The fight, step by step
- Finish Whispers in the Walls, then pick Effervo on Deimos.
- Find an Atropos Probe and inject its serum into a Vitreum to start a scan.
- Defend the scanner while it looks for Murmur Eyes, and collect them. 30 are needed on the normal path.
- When you have enough, the boss location is revealed. You will meet one of three variants: Suzerain, Zelator or Anchorite.
- Fight the variant and wait for it to show glowing teal spheres during an attack. Shoot them for extra damage and a crit tier.
- Use Radiation or Electricity. Viral does little here.
- On the Steel Path, collect 60 Eyes to face the Fragmented One, the hardest form.

Tips that save runs
- Squad up for speed. Big area weapons make the eye scan and the boss adds trivial.
- The teal spheres show up during specific attacks, so wait for the tell rather than spraying.
- Do not rush the Steel Path Eyes. Keep the scanner alive.
Mistakes that waste runs
- Leaving the Vitreum undefended and losing scan progress.
- Using Viral damage against it.
